Registration Info for Teams in the Tournament
If you have been awarded a bid to the 2006 UPA Youth Club Championships,
congratulations! This tournament promises to be an exciting and unforgettable
weekend for you and your team. But before the fun begins, it is important that
we take care of some pre-tournament business. Please read the instructions below.
All teams are required to complete all administrative paperwork completely and
on-time before they will be allowed to participate in the tournament.

Due Date
All paperwork and fees must ARRIVE (not just postmarked) at UPA Headquarters by Friday, July 14th.
There will be a $50 late fee for teams who submit rosters and fees after that deadline.

Tournament Fees
$25/player - Checks should be made out to "UPA".
Please note on the check and/or in a note that this is the "YCC tourney fee" (as opposed to UPA dues).

Roster -
All players attending the tournament must be listed on an official 2006 UPA Youth Club Championships roster.
You may use the Excel or PDF rosters, found at the link listed above, or if you wish to use the online
rostering system to create your roster for the event, please email

for instructions.
Either way, you must submit the required money and paperwork outline on this page by the July 14th deadline.
UPA Membership Dues -
All players attending the tournament must be UPA members.
UPA dues for Youth Memberships for 2006 are $20/person.
You can pay by check (made out to "UPA") or with a credit card by following
the instructions on the UPA website
(http://www.upa.org/members).
(One-time event fees are not sufficient for participation in this event.)

Waivers -
All players must have a valid waiver on file with the UPA.
Waivers are good for the calendar year in which they were signed.
The same waiver is good for minors and adults (>18), but minor waivers must
also be signed by a legal guardian.
Medical Release Form -
All minors (players under 18) must return a copy of a completed medical release form.
This form empowers a person or group to authorize medical care for a minor while at
the tournament. The form must release the minor player to the group the team is
representing or one of the official chaperones for the event. Chaperones should bring
the original medical release forms to the event.
Chaperone Waiver -
Each attending team must have 2 chaperones who are at least 25 years old.
All chaperones must fill out and return a chaperone waiver.
Team Info Form -
The Team Info Form
must be completed by each attending team and emailed to the UPA by July 14th.
